CREDS services are available to those living in the Central Region of New Zealand – the Wellington, Hutt Valley, Wairarapa, Palmerston North, Hawkes Bay and Whanganui DHB regions.

For those living in the Wellington region

To refer a client to CREDS, the first step is completing our Self Assessment Form, and Pre-Assessment Medical Review Form. The information gathered by these forms gives us a good insight into the client’s situation, and will help us to determine which (if any) of our services will be the most appropriate for them.

Once we have received the completed forms, we will contact the client to arrange a time for them to come in and meet with a member of our therapy team for a face-to-face assessment. We will then discuss the treatment options available to them.

It is important that we let you know that we have a waiting list for our services. This varies depending on the service.

For those living in the Palmerston North, Hawkes Bay or Whanganui regions

In order to access CREDS services, we need to work collaboratively with your local Community Mental Health Team (CMHT). Before referring your client to CREDS, please refer them in the first instance to their nearest CMHT.

The following forms are required to access CREDS services from your region:
